2 Revisit Patients

  • The revisit reception machine cannot be used if your health insurance certificate has not been checked by our hospital in the past two months. Present the certificate at the Revisit Counter (No.2).
  • A revisit is handled as a first visit after an absence of six months or longer (except for vaccinations, health checkups and other services that are not covered by health insurance plans).
  • Fill out the registration form and submit it to the First Visit Counter (No. 1) together with your health insurance certificate and any other medical certificates.(If you have a letter of referral from another medical institution, submit it as well.)
  • Note in advance that if you have not visited our hospital for six months or longer since your last visit, you will be handled as a first-visit patient, and will need to pay 3,240 yen for Special Medical Care Coverage.

3 How to use the return visit reception machine

  1. Insert your patient ID card into the slot.
  2. Press the button of your department.
  3. Press the button of your treatment
    (examination required / medication only / injection only).
  4. Press the "End" button.
  5. Take the acceptance slip and your patient ID card.
  6. If the instruction "Present your health insurance certificate during today's visit" is indicated on the acceptance slip, do so at Counter No. 2.

4 For a visit to two or more departments

Repeat Steps 2 and 3 above before pressing the "End" button. Start the steps with the department you would like to visit first. Also start the steps with a department for medication only (if any).

5 When you arrive at the hospital earlier than the start of reception

- If you have visited our hospital before

Morning reception

Sit in one of the sofas in front of the pharmacy in the central lobby, in order of arrival. Starting at 8:30, we will guide you to the return visit reception machines.
(If your health insurance certificate has not been checked by our hospital in the past two months, be sure to present the certificate at the Return Visit Counter (No.2) in advance.)

Afternoon reception

Take a number at the Return Visit Counter (No. 2).
Starting at 12:30, our staff will collect your number in order of issuance and guide you to the return visit reception machines for check-in.
(Numbers are issued starting at 8:30.)

6 Patient ID card and health insurance certificate

Your health insurance certificate and medical certificates should be checked every month. At your first visit

of the month, check in using the return visit machine and present your health insurance certificate and medical certificates at Counter No.2.

Only one patient ID card is issued for each patient, and is common to all departments. Be sure to bring it

when you visit our hospital.

If your patient ID card becomes lost, it should be reissued at your expense.

Please note that you may be required to pay the entire account by yourself if your health insurance certificate

has not been presented for two consecutive months.

7 Making appointments

Appointment for a return visit

When you need a return visit, an appointment is made following your examination. Please consult your doctor.

Changing an appointment

Your appointment can be changed over the phone.
Contact your department up to the day before the planned examination if possible.
(Calls are accepted from 10:00 - 12:00 and 14:00 - 16:00, Monday through Friday.)
For more details, contact reception at your department.
* Please note that you may be required to wait for some time even though you have an appointment, depending on the examination schedule of earlier patients.

8 If you have visited our hospital before

For a visit to a department that you have never consulted before:

Check in by yourself at the First Visit Counter (Counter No. 1). Fill out a registration form and present it at the First Visit Counter (Counter No. 1), together with your health insurance certificate and/or other medical certificates. (If you have a referral letter from another facility, also present it at the same time.)

For a visit to a department that you have consulted before:

YCheck in by yourself using a return visit reception machine. The use of a return visit reception machine may not be available depending on the hour or department. In that case, check in by yourself at the Return Visit Counter (Counter No. 2).

9 Special or Specified Medical Care Coverage (patient-selected extra medical services)

For a visit to a department that you have never consulted before:

Check in by yourself at the First Visit Counter (Counter No. 1). Fill out a registration form and present it at the First Visit Counter (Counter No. 1), together with your health insurance certificate and/or other medical certificates. (If you have a referral letter from another facility, also present it at the same time.)

For a visit to a department that you have consulted before:
